1. 2010 National Pop Warner Player Registration Form
(players name must match birth certificate exactly)
2. 2010 National Pop Warner Proof of Physical Fitness
(dated after January 1st 2010 and signed by physician)
3. 2010 All American Pop Warner Scholastics Form (Sign Only)
4. 2010 Almaden Pop Warner Registration Form- SIGNED 2 sides
5. Copy of Birth Certificate(Xerox Copy- No Originals)
6. Copy of complete Year-End report card for 2009-2010
7. 2x3 Head shot photo— NO Baseball Caps or Sun Glasses
8. Copy of Insurance card
9. Volunteer Sign up or $250 Buy-Out option paid in full.
10. $390 Registration fee paid in full ( If required )
11. $250 equipment deposit.
