​The plan consists of a $50 deposit and 4 equal payments for the balance.  Payment plans end July 1.

REFUND POLICY

We recognize that some participants may have to withdraw from the Almaden Mustangs for various personal reasons. All refunds are handled on a case by case basis and are at the discretion of the AVYAA Board of Directors. We will never reimburse for custom purchases and no refunds are guaranteed.

RETURNED CHECK FEE

A fee of $25.00 will be charged for all returned checks.
 

REGISTRATION PAPERWORK DAY - TBD

This is the day that all of your athletes' required documents must be turned in.  
Athlete's are not allowed to begin practice without a physical. No exceptions.

AUGUST 1 - practice begins.

Please bring ALL of the following items with you.

  1. Standard Participation Contract: players name must match birth certificate exactly.

  2. Proof of Physical Fitness: TWO SIDES - ORIGINAL copy must be dated after January 1, 2018 and signed by physician - NO COPIES

  3. Birth Certificate  - 1 copy

  4. Equipment and Volunteer Contracts and two checks in the amount of $250 for equipment and $175 for volunteer hours. Both checks will be held, not cashed unless equipment isn’t returned, hours aren't complete
    or a balance isn't due. Upon the end of the season your checks, if not cashed for any of the reason listed, will be returned. If we are unable to return, we will shred the checks. 

  5. (2) Copies of COMPLETE Year-End report card for 2018-2019 (Scaled to fit on 8 1/2 x 11)

  6. 2x3 Head shot photo - NO hats or sunglasses

  7. Volunteer Buy - Out option $175 paid in full

  8. Registration fee paid in full, if not already paid
